Check your transition readiness

See how well-supported your routines are across eight areas. The result is More support recommended or Ready for a clinician conversation.

The readiness index scores eight areas of your own data to show where your routines are well-supported and where they aren't. It is not medical clearance. It never says whether stopping, changing or continuing your medication is safe — that decision belongs to you and your clinician.

Steps

  1. Open the Transition Center and select Check now on the Readiness card, or go to Readiness.
  2. Read the result at the top. There are only two possible results: More support recommended or Ready for a clinician conversation.
  3. If more support is recommended, Where to focus lists your three weakest areas. All eight areas shows every score with an explanation.
  4. Under Your own read, set How ready do you feel? and tick I have a clinician I can actually reach about this if that's true. Select Update.
  5. Select Save this to my record to add today's result to your trend.

Good to know

  • Ready for a clinician conversation means your routines look well-supported. It's a conversation to have with your clinician, not a decision to make alone.
  • Any single area scoring below 50 keeps the result at More support recommended, whatever the average.
  • Your own confidence counts for an eighth of the score.
  • Checking is free and changes nothing. Only saving adds a point to the trend on the Transition Center, which a summary for your clinician can include.
