Prepare for my appointment

Build a one-page summary of your doses, symptoms and trends for any clinician, choosing what goes in before you share it.

Prepare for my appointment builds a one-page summary of what you've recorded — doses, symptoms, routines and trends — for your prescriber or any clinician you see. You choose what goes in and see it before anything is shared. Nothing in it is interpreted, and nothing recommends a medication change.

Steps

  1. Select Prepare for my appointment on Today, the Medication page or Your journey, or New packet under Clinician packets in the Transition Center.
  2. Under Which summary, leave For a clinician selected — or choose Engagement only — for a plan or employer if what you need is proof that you're taking part.
  3. Under What to include, pick a Period (30, 60, 90 or 180 days; 90 is the default) and tick the sections you want.
  4. Open See exactly what your clinician will see to check the preview.
  5. Under Consent, add any Questions you want to raise. They appear word for word.
  6. Tick the consent box and select Create summary. Your finished summary opens, with its share link.

Good to know

  • An Engagement Summary carries participation only — days with something logged, check-ins, lessons, routines held, strength sessions — and cannot carry a dose, a symptom, a weight or a lab result: those sections don't exist on it. It's for a plan or employer that ties coverage to taking part. You hand it over yourself; GLP-Won never sends it to anyone, and your employer's console can never read it.
  • If you've recorded a clinician-directed transition and picked the route your clinician chose, a clinician summary shows that route and its date under Clinician-directed transition. Their words themselves stay on your Transition page.
  • Weight starts unticked if you've hidden it in Settings. You can still include it for this one summary.
  • Strength, function, activity and sleep includes your first and latest strength estimate for the lift you log most, each functional test you've taken and your waist, within the period you picked.
  • The Transition readiness, rehearsals and plans section is offered once you've started exploring readiness or reported a lapse.
  • A summary is frozen when it's created: what your clinician reads later is exactly what you shared that day.
  • Nothing is created until you tick the consent box.
